Instructions

Follow these instructions to complete the Tuition Assessment Form. You can present the form in person, by faxing it to (520) 515-5452 or emailing it to registration@cochise.edu or admissions@cochise.edu.

  1. fill out the assessment form
  2. print and sign the page
  3. submit the form with all the documentation
